We would like to inform our patients that unfortunately Dr Fontaine will be taking a leave of absence for the foreseeable future.

Although we will endeavour to seek locum support where possible, it is likely that our available appointments and the level of access to clinicians that we usually deliver will be adversely affected by this. Please understand that we may not be able to offer our usual level of access during this time.

Our receptionists may refer you to an urgent care centre in Banbury if you need an urgent appointment and we are unable to provide one.

If you are having musculoskeletal (MSK) problems you can self-refer to a physio here https://www.connecthealth.co.uk or ask our receptionist to book you a Saturday morning physio appointment.

If you have sinusitis, shingles, sore throat, impetigo or a UTI, amongst other conditions, you can seek help at a local pharmacy. More information as well as how to find a pharmacy can be found here https://www.nhs.uk/nhs-services/pharmacies

If you are pregnant, please contact the midwives at Horton hospital or Marie Stopes if you are considering of termination of pregnancy.

If you have an eye problem, please contact an Oxfordshire optician who offers a Minor Eye Conditions service (MECS)

We will keep you as up to date as possible, and we thank you in advance for your ongoing support and understanding.

The NHS have arranged several 'Protected Learning' days throughout the year. These days provide time for staff to complete mandatory training which is essential to maintain a safe and effective environment for patients. During these times the surgery will be closed. The next day for one of these is Tuesday 12th November.

Usually this learning time will take place over an afternoon, so in the mornings we will be open as usual.

Due to necessary changes in response to NHS initiatives surrounding online access, patients who use online access or patient services online may experience disruptions to their access. Please contact the surgery if your level of access changes.

New Patient Registration

Our practice area is comprised of the area between Banbury, Chipping Norton and Shipston. It encompasses various villages such as Brailes, Tysoe, Honington, Stourton and Hook Norton. We do not currently accept new registrations from Banbury, Chipping Norton or Shipston. If you're unsure whether or not you are within our area, please feel free to ask.

When registering, we will need to ask you for identification, this could be a passport, birth certificate or driving licence.

Although you will be registered with the Practice rather than a specific doctor, should you prefer to see either of the doctors then please let us know and we can record this on your registration.

You will be given a medical questionnaire. Medical treatment is available from the date of registration. Please contact reception for further information.

New Patient Online Registration

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
A paper form is still available if you need one.

They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
