As part of the NHS's efforts to protect you and health workers at this difficult time, Oxfordshire GP practices have changed the way they work in your area.

 

The most important thing you can do to stay safe, protect your family and protect your NHS doctors and nurses is to follow public health advice to stay at home and keep your distance from other people.

PLEASE DO NOT VISIT THE GP SURGERY IN PERSON. If you need health advice for suspected coronavirus, contact NHS 111 online or by phone. You can also contact your GP practice through its online consultation service or by telephone if you are concerned about coronavirus or any other health condition or symptom. You will be offered a telephone appointment or you will be advised if you need to be seen by a doctor, and safe arrangements for this will be made.

Please be aware that your usual doctor, nurse or other health professional may not be available and you may have to travel to another location for face-to-face care if that becomes necessary.

A volunteer based delivery service for medication has been organised by members of our community. If you are unable to attend the surgery to collect your medication, please contact us and ask about the service.

WE WILL GIVE YOU A LOGIN WHICH YOU WILL USE TO REGISTER. YOU CAN THEN BOOK APPOINTMENTS AND ORDER REPEAT PRESCRIPTIONS FROM YOUR LAPTOP, TABLET OR SMARTPHONE

New Patient Registration

If you live within our practice area and would like to register with us, please bring your NHS card along to the surgery. If you have lost your NHS card then the receptionist will give you an alternative form to complete. We will need to ask you for identification to copy and hold on your file, this could be a passport, birth certificate or driving licence.

 

Although you will be registered with the Practice rather than a specific doctor, should you prefer to see either of the doctors then please let us know and we can record this on your registration.

 

You will be given a medical questionnaire. Medical treatment is available from the date of registration. Please contact reception for further information.
